Filing a request for arbitration, also called application for arbitration or notice of arbitration, is the first step to initiate an arbitration. A request for arbitration shall contain:
(1) the basic information of the parties, including the name, sex, age, occupation, employer, and domicile of each party in the case of an individual, and the name, domicile, and legal representative's or principal's name and title of each party in the case of a legal entity or other organization;
(2) specific claims and reliefs sought, and the facts and grounds as the basis for such claims and reliefs;
(3) valid evidence and their sources; and
(4) basic information of the witnesses, including but not limited to their names, domiciles, contact information, etc..
